2. Form-1 (aa) Format of extended Producer responsibility authorisation.
3. Form-1 (bb) Grant of authorisation for generation or storage or treatment or refurbishing or disposal of E-waste by Manufacturer or Refurbisher.
4. Form-4 Application Form Authorisation of Facilities possessing Environmentally Sound Management practice for Dismantling or Recycling of E-Waste.
5. Valid Consent to Operate Order from the SPCB under Air & Water Act.
6. Authorisation fees – Nil
7. Land details with Layout map.
8. Agreement with Recyclers in case of Collection Centre and Dismantler.

Fees : No Fees is applicable for the E-Waste.
