   ii.Form-II for Plastic Waste Recyclers
   iii.Form-III for Manufacturer of Plastic Raw Material
2. Copy of DIC Registration Certificate.
3. Valid Consent to Operate Order from the SPCB.
4. Requisite Fees.
5. Action Plan Endorsed by State Urban Development Department.

Fees : Rs 2000 per annum.

Note : The fees shall be levied towards the registration fee from the producer / manufacturer & recycler of plastic virgin carry bags / multi-layered plastic packaging products / recycled plastic bags / plastic sheets / sachets.
