General Instructions
All applications from eligible applicants are received by the concerned Drug Inspectors (DIs) at the respective districts. The DI scrutinizes the application and conducts site verification. If the documents submitted by the applicant are in order and site verification report is satisfactory, the application is forwarded to the Licensing Authority for issuance.
Fields marked with ‘*’ are mandatory and should be filled in before submitting the form on the single window portal. Leaving fields blank in the Form is not allowed. In case applicants wish not to enter data in a field, they can put “NA” if it is a text/description field or a “0”, if it is a numeric field. In case of errors, the form will show an error message. The applicant may correct the data and save the form again.
Some of the fields can accept multiple values. If the applicant wishes to furnish more than one item, he may use the “+” button to add more rows. Similarly, if he wishes to remove a row, he may use “x” button.
Applicants can attach documents in digital format as documentary evidences which the department will use for verification and issuance of the license. Clicking on the ‘Choose’ button will allow them to select documents to be uploaded. Before submitting the application, applicants need to ensure that all the information furnished by them is satisfactory and correct to the best of their knowledge and understanding. The application can be submitted by clicking on the ‘Submit’ button.
After the form is successfully submitted, applicants can pay the required license fees online. Upon processing of payment by the bank, an SMS & Email notification with the application number is sent to the applicant on the registered mobile number and email address. The application number will be used for tracking the application online.
Checklist of Documents
1. Educational Qualification certificate of applicant(s) (Managing Partner/Power of Attorney Holder for Partnership Firms and Managing Director/Authorized Signatory for Pvt. Ltd. Firms)
2. Educational Qualification certificate of Registered Pharmacist and valid O.P.C. registration certificate (self-attested by applicant with full signature and date)
3. ID Proof of applicant
4. Purchase bill or invoice of Refrigerator issued in favour of the firm/applicant
5. Specimen signature of Registered Pharmacist which shall appear in the sales invoices, prescriptions and special register
6. Tenancy/house rent agreement certified by Notary
7. Patta or Khatian or documentary evidence in support of ownership of land and NOC from Co-house owner (wherever applicable) self-attested by applicant with full signature and date
8. Location and layout sketch map of premises duly signed by applicant as well as house owner
9. Character and Antecedent verification Certificate or Police verification certificate of applicant
10. Self-Declaration of Registered Pharmacist in the prescribed format with full signature and date
11. Registered Partnership Deed (for Partnership Firms), duly registered at Inspector General Regulation, Cuttack or at District Registrar/Sub-registrar Office
12. Letter of authorization from Partners for filling application as Power of Attorney Holder/Managing Partner(s) (for Partnership Firms)
13. Memorandum and Article of Association with company Registration Certificate (for Pvt. Ltd. Firms)
14. Resolution of Board of Directors (for Pvt. Ltd. Firms)
15. Letter of authorization from Directors for filling application as authorized signatory/ Managing Director(s) (for Pvt. Ltd. Firms)
Timeline
Retail Drug License (Pharmacy) and renewal thereof is provided within 40 working days from the date of submission of application. The department can however, raise queries on the application within 7 working days of receipt of application.
